Organization Details

Our program services include the following: - Full time support from a nurse practitioner and a community health worker staffing the Baruch Houses Family Health Center (that can provide case and disease management to patients of the health center but can also provide case management to residents of Baruch Houses who receive medical care in other health centers/ hospitals) - Additional support from 8 resident outreach workers - Free health workshops on topics such as lifestyle changes, chronic disease self-management, and health insurance screening - More free pubic events to promote health (open to the general community, not just residents of Baruch Houses).
