Hospice is a specialized program of care for patients with end-stage disease. Continuum Hospice Care Hospice is a specialized program of care for patients with end-stage disease. Continuum Hospice Care believes that individuals should have the opportunity to prepare for the end of life in a way that is physically, emotionally, socially and spiritually supportive. Hospice provides compassionate care for the patient, and support for their family and caregivers, so that they may live as fully and comfortably as possible. Interdisciplinary services and symptom management are provided by a team that includes doctors, nurses, social workers, chaplains, volunteers and bereavement counselors. The team works together to help patients and their loved ones with their physical, emotional, social and spiritual needs. Hospice care is delivered to the patient at home, at a hospice inpatient unit or residence, at a long-term nursing facility or a participating hospital. The majority of patients are cared for in their own homes. Ninety percent of our patients receive care in their own homes, but if that is not possible and the patient does not require the acute care available in the Jacob Perlow Hospice inpatient unit, the Mollie and Jack Zicklin Hospice Residence is available.
